Why do I have to disclose my identity when I send documents by means of the document transmission form?
This enables us to process the information adequately and to match the documents with the complaint file. If you are a government department or health and social services employee, please add your title, extension number, given name and surname. This will facilitate processing of the file.
When I send documents concerning my file by means of the document transmission form, what information do I provide?
When you send documents by means of the document transmission form for a file that is being processed, it is preferable to enter your file number and the name of the person to whom we must send the documents in the fields for that purpose.
This information is useful in order for your request to be handled properly. If you do not remember your file number or the person’s name, you do not have to complete these fields.
You can also use the available space to add any extra information or notes that could help us process your request.
What happens once the Québec Ombudsman has received my online complaint?
As soon as the Québec Ombudsman has received your complaint form, it acknowledges receipt by contacting you by phone within the following two working days.
Then, if your complaint falls within the Québec Ombudsman’s jurisdiction (in other words, if the Québec Ombudsman can intervene), it begins its investigation of all the organizations concerned.
When the conclusions arising from the investigation have been reached, it will inform you either in writing or verbally, regardless of whether the complaint was substantiated or unsubstantiated or whether or not recommendations were made.
If your complaint does not fall within our jurisdiction and, as a result, we cannot process it, we will help you find the appropriate resource or recourse.
What happens when I click on SEND THE FORM?
Your complaint is transmitted to the Québec Ombudsman and a message confirming that it has sent is displayed on the screen. You can print this confirmation (by printing the Web page from your browser). You can also save a copy of the form you just sent for your records.
I have an international phone number that does not match the number of boxes in the contact information section of the form. What should I do?
In the summary of your complaint (STEP 2 of the form), specify that the phone number is international and indicate the number. Then at Step 3 of the contact information, enter a series of ten digits (e.g. 000 000 0000) in the mandatory field to be able to go on to Step 4.
Can I go back to edit a section of the online form that is already filled in (before sending it)?
Yes. As long as you have not sent the online complaint form, you can go back to the preceding section by clicking on PREVIOUS STEP to edit the fields or add information.
At STEP 4 (confirmation), the entire completed form is displayed. You can still edit the entries for any of the steps and save your changes before sending the form.
How can I be sure that the personal information I transmit online will remain confidential?
To protect your personal information, our online complaint form is secure. The information sent online is rigorously encrypted to prevent pirating.
What’s more, your complaint will be handled with complete confidentiality and respect. The Québec Ombudsman conducts its investigations privately, meaning that personal information is always protected and only the people authorized to consult it in exercising their duties have access to it.
I can’t seem to be able to attach documents to my complaint form or document transmission form. Why?

Check the format and size of your documents
To ensure secure transmission, the web form automatically blocks files that could be dangerous or corrupted or that are too large to send. Note that the combined size of all attached documents must not exceed 75 MB. Each file must not exceed 5 MB.
Only files ending in the following extensions are authorized: .avi .bmp .cda .doc .docx .docm .dot .gif .htm .html .jpeg .jpg .mht .mp3 .mp4 .mpeg .mpg .pdf .png .pps .ppsx .ppt .pptm .pptx .rtf .tif .tiff .txt .wav .wma .wmv .wpl .wps .xls .xlsm .xlsx and .xps.
