Call-back form

You can choose a call-back date from among the next three available business days by clicking on the icon on the calendar.

Once you have chosen the day, a drop-down menu of the available time slots will be displayed to the right of the calendar. Select the time slot for the call-back call.

Note that because of high demand, certain time slots may be unavailable and the dates of the three available business days may fluctuate.

Note: This form is for use only if you are contacting the Québec Ombudsman for the first time.
If you’ve already sent a complaint form, please don’t use the call-back form. For any follow-up on your file that is being processed, please call us directly at 1-800-463-5070, Monday to Friday from 8:30 a.m. to 4:30 p.m.
